Twins Probability Calculator

A gentle statistical estimate of twin-pregnancy chances based on maternal age, family history, previous pregnancies and fertility treatment.

Twins are special, and the odds depend on several factors. This calculator combines the factors that affect fraternal (non-identical) twinning, adds the stable identical-twin rate, and shows a realistic range with a plain-language explanation.

Estimate twin chance

Fraternal twins on the mother's side?

This means a mother, sister or other close relative on the mother's side.

Has the mother had a twin pregnancy before?
Optional factors

These have a small effect. Leave them on "Not sure" if you prefer.

Your result

This is a statistical estimate, not a diagnosis. Only an ultrasound can confirm a twin pregnancy.

How it works

Twin pregnancies come in two main types. Identical (monozygotic) twins happen when one fertilised egg splits, at roughly 3–4 per 1,000 births. It is treated here as independent of age, family history and fertility treatment. Fraternal (dizygotic) twins happen when two eggs are fertilised, and that rate is much more sensitive to the factors in this calculator.

The model starts with a total twin rate of about 32 per 1,000 births. It subtracts the identical component, then applies multipliers to the fraternal component only. The multipliers cover maternal age, fraternal twins on the mother's side, a previous twin pregnancy, previous pregnancies, fertility treatment, and optional height or BMI categories. Fertility treatment has the largest effect. The final estimate is capped so it stays in a realistic range.

Age has a curved relationship. Fraternal twinning tends to rise through the 30s and peak in the late 30s, then fall after 40. Family history on the mother's side matters more than the father's side. A previous twin pregnancy and more previous pregnancies also raise the estimate. Taller mothers and some BMI categories are linked to slightly higher fraternal twinning, but the effect is modest.

This is a statistical estimate, not a diagnosis. Only an ultrasound or another medical assessment can confirm a twin pregnancy. If you are pregnant and have questions, your maternity team is the right place to ask. Everything is calculated in your browser; we do not store your answers.

Frequently asked questions

How common are twins?

Roughly 30–33 out of every 1,000 births are twins in many high-income countries, though rates vary by place, age and fertility treatment. Identical twins make up about 3–4 per 1,000 births. The rest are fraternal.

Does family history really matter?

Fraternal twinning runs in families, especially on the mother's side, because it is linked to how many eggs are released. Identical twinning does not appear to run in families in the same way. The calculator applies family history only to the fraternal component.

Why does maternal age have a curved effect?

Fraternal twinning tends to rise with maternal age through the 30s, peaking in the late 30s, and then falls after 40. The calculator uses age bands to reflect that curve rather than assuming that older is always higher.

Do fertility treatments increase the chance of twins?

Yes, fertility treatment can raise the chance, especially when more than one embryo is transferred or ovulation is stimulated. The effect is larger than most other factors. Modern single-embryo transfer reduces the risk, so your clinic's advice is more specific than any calculator.

Can this calculator tell me if I am having twins?

No. It estimates a probability from general factors and cannot detect a twin pregnancy. An ultrasound is the usual way to confirm twins. Speak with your maternity team if you have symptoms or questions.

Is my data stored?

No. The calculation runs in your browser. We do not send your age, family history or fertility answers to our servers.
